Your first visit at Valley Oral Surgery

What to bring

Please assist us by providing the following information at the time of your consultation.

If your dentist or physician has provided you with current x-rays, please bring them to your consultation appointment.

Digital radiographs, along with your dental referral, can be e-mailed to our office at VOSClinical@valleyoralsurgery.com.

Surgical referral slip if available (a referral is not required).

A list of medications you are currently taking.

Your current medical and dental insurance information.

Consultations

Each patient receives a consultation to evaluate his or her concerns. This includes a clinical examination with review of medical history and current x-rays. Diagnoses are discussed in detail and treatment options are thoroughly reviewed. A personalized treatment solution is then selected for each patient.

Imaging and x-ray services

Imaging services are an integral part of diagnosis and treatment. Our practice offers the latest advances in dental and maxillofacial radiographic imaging including panoramic x-rays and cone beam CT scanning. Appropriate imaging will be recommended for your specific case.

Treatment of minors

All minors (under the age of 18) seeking treatment must be accompanied by a parent/legal guardian.
